Come to the Elementary Office next week Monday, Tuesday, Wednesday or Friday between 9:30 AM and 4:00 PM and on Thursday from 9:30AM-7:00PM to register your 2021-2022 kindergarten student.
If you don’t have all of the documents needed, don’t worry! Go ahead and bring what you have.
When you register your student, you will sign them up for their kindergarten screening appointment on May 28, June 1 or June 2.

Child must be five years old on or before July 31, 2021 to be eligible to start Kindergarten.
1. Complete online registration at https://www.muschools.com/o/elementary-school Go to Parent Links Select KINDERGARTEN REGISTRATION (2021-2022)
2. The following documents are required for enrollment. Missing or incomplete documents will result in a delay of enrollment.
* Original Birth Certificate
*Proof of Residency (e.g., signed lease, utility bill, court document, etc.)
*All Special Ed/504 Documents
*Parent/ Guardian's Valid Driver's License
*Custody Papers
**Notarized Residence Statement (Signature must be notarized. You can do this at the Elementary Office if you bring your driver’s license with you.)
**Medical Record (Must be completed and signed by physician)
The above forms with ** can be picked up in the Elementary Office. All other documents should be brought to the Elementary Office during the hours listed below:
Monday, April 26 9:30 AM – 4:00 PM
Tuesday, April 27 9:30 AM – 4:00 PM
Wednesday, April 28 9:30 AM –4:00 PM
Thursday, April 29 9:30 AM – 7:00 PM
Friday, April 30 9:30 AM – 4:00 PM
